Care Plans

  • The Chronic Disease Management (CDM) care plans, formerly EPC’s, is a Medicare initiative that allows eligible individuals to access up to 5 Medicare Subsidised allied health sessions per calendar year, including physiotherapy.

  • We support clients under these plans with in-home physiotherapy to help manage pain, maintain mobility, and improve day-to-day function.

  • You may be eligible for a care plan if have a chronic or ongoing medical condition that has lasted (or is expected to last) for 6 months or more. Common conditions include arthritis, persistent muscle injuries, diabetes, osteoporosis, chronic pain, heart disease, stroke, chronic respiratory conditions, neurological conditions.

Discounted rates for concession card holders

We’re committed to keeping physiotherapy accessible. If you hold an eligible concession card, we offer reduced out-of-pocket costs. Eligible cards include:

  • Pensioner Concession Card

  • Health Care Card

  • Low Income Health Care Card

FAQ's

  • How much does it cost?

    Medicare covers part of the cost and you pay the gap. The gap depends on your circumstances as we offer discounted rates for eligible concession card holders.

  • Can I use all 5 sesions for physio?

    Yes, if your GP refers you specifically for physiotherapy, you can use all 5 sessions with us.

  • Can I choose my provider?

    Yes, you have the right to choose your preferred provider. We’re happy to work alongside your GP and other health professionals as part of your care team
